Polycom VVX 411 Failing to Provision
Resolution steps:
1. First you will need to look at the back of your phone to
locate the MAC address. This will be a 12-digit number appearing on a sticker at the back of your phone. Write this down somewhere as you must enter it as the reset password later.
2. Once you have this address, follow these steps to initiate the reset:
- Power off the phone by unplugging the power or network
- Plug the phone back in
- Wait until you see Cancel, then press Cancel button
- Quickly press and hold down the following keys: 1,3,5
3. After holding down the correct keys during boot, you should be prompted for an admin password.
4. Enter the MAC address of the phone as the password. To enter the password, use the keypad to type it in. You will need to switch between numeric and alphanumeric entry modes to enter it correctly.
5. Once the password is entered correctly, you will be prompted a final time whether you would like to reset it. Proceed and your phone should now reboot to complete the process.
6. If you get the password wrong, on the next screen, you can simply press Cancel and it will return to the password prompt and you can try again.
7. Open your web browser and open the phone web interface by typing in the I.P address assigned to the phone e.g if the I.P address is 172.16.14.24 then type
https://172.16.14.24 in the address bar and log in as Admin using

8. When logged in, navigate to
Settings and click on
Provisioning Server and change the server type to
HTTPS and type in
p.wprov.net as the server address, and save.
9. Navigate to Settings and click on Network and select Ethernet and set the IP mode to IPV4 and disable DHCP then enter the I.P address currently assigned to the phone. Make sure you also enter the subnet mask and gateway congruent with the VLAN the device is connected to.
10. Navigate to the
General Ethernet settings and type in the
Google DNS as the primary DNS server and the VLAN gateway as the
alternate server.11. Click save and the phone should automatically reboot and provision.
